Parent - Student Policy Signature Form

The second quarter of school is almost here and your child has enrolled in the Certified Nursing Assistant class to be held at Reuther beginning November 7, 2005 from 7:45-10:45 a.m. There are a few requirements for the Nursing Assistant class that we want you to be aware of so you can help your student be successful. 

  1. Attendance:  the student may miss only 12 hours of class including clinical.  Do not schedule appointments such as Dr., dentist, Soc. Serv., W.I.C., etc. during class time. Students missing more than 12 hours of class do not pass. Parents of students who do not pass will be required to re-pay the school district for the cost of the class.
  2. We need a copy of their social security card and state ID or Drivers License in order to submit their name for state testing and the state registry.  Please bring these the first day of class.
  3. A Background Information Disclosure Form is required by Gateway and the State of Wisconsin for anyone entering the health care field.  This requires honest responses to questions about previous crimes and offenses that may bar or restrict employment in this field. Gateway would also like you to complete their application form before the start of the class. We will have these the first day of class if they are not already on file at Gateway.
  4. A physical exam is required before the student can go to the clinical setting (nursing home).  The Gateway physical form will be supplied by GTC and must be signed by a physician. The exam must include a TB skin test.  The forms must be turned in one week prior to the start of clinical. Those with incomplete physicals will not be allowed to attend clinical. Please make an after school appointment now with your physician or clinic for the physical.
  5. A basic uniform is required for the clinical experience. The uniform is needed for the clinical practice at the end of the quarter so you do not need to purchase it now. We will discuss this in class and provide options for purchase.
  6. The classroom portion of the program must be passed with a grade of 78 or better.  The clinical portion is a pass or fail.  The student must be able to perform the skills learned in class on real patients during clinicals at a nursing home.  The state contracts with an outside agency to test the students for the Registry.  The date and time of these tests is to be announced. The American Red Cross does testing. The fee for testing is $100.  Testing is optional but necessary to work.
  7. Should enrollment exceed the number of students allowed, we will need to reduce the number enrolled. This decision will be based on a number of factors including date of enrollment, grade in school, and attendance records.
